PET REQUEST FORM — RENTERS’ RIGHTS ACT
One form must be completed for each individual pet. Applications that are incomplete or submitted without the required supporting evidence cannot be assessed and will be returned for full resubmission. Submitting this form will open your email client, allowing you to attach all required documentation and send your application in one complete submission.
IMPORTANT — READ BEFORE COMPLETING
Under the Renters’ Rights Act, tenants may request consent to keep a pet. Each request must be assessed on the information provided.
We will only consider applications that are:
  • Fully completed
  • Supported by documentary evidence
  • Submitted in one single submission (one email submission with all supporting evidence attached)
Multiple or piecemeal submissions for the same pet will not be considered.
If evidence is missing or sent separately, a new application must be completed and resubmitted in full.
We will respond in writing within the required timeframe once a complete application (with evidence) is received.

Will the pet be regularly flea/worm treated? *
Will the pet have adequate exercise/mental stimulation? *
5) Care & supervision arrangements
Will the pet be left alone for extended periods? (anything that exceeds 1-2 hours) *
This becomes required if you selected “Yes”.
6) Property use & suitability
Access to outdoor space (if applicable) *
Will the pet be kept in communal areas on a lead / in a carrier (if applicable)? *
7) Supporting evidence (tick one per item)
Applications will only be assessed on evidence provided.

For each item below, select Yes (evidence attached), No, or N/A as applicable.
Selecting “Yes (evidence attached)” means you will attach that evidence to the email submission. If you select “Yes” and do not attach evidence, the application may be rejected as incomplete.
 
Microchipping *
Microchipping may be a factor in refusing consent where legally required/applicable and not complied with.

9) Conditions of consent & declaration
If consent is granted, it will be: pet-specific and property-specific, and subject to the conditions below.
  • The pet will not be allowed to cause nuisance/annoyance (including excessive noise, fouling, or aggressive behaviour).
  • Any pet-related damage/extra cleaning/flea treatment required will be paid for by the tenant (and may be deducted from the deposit with evidence).
  • The tenant will keep the property reasonably clean and will ventilate appropriately to reduce odours/condensation.
  • The tenant will not keep additional pets without fresh written consent (pet-specific consent).
  • The tenant will notify Prime Choice if the pet changes (e.g. additional pet, replacement pet, breeding, or long-term pet sitting).
Reminder: If you have more than one pet, you must submit a separate form (and separate email) for each pet.
